The decision between preserving a compromised tooth and replacing it with an implant, especially in the esthetic zone, remains one of the most complex clinical dilemmas. Borderline situations require careful assessment, integrating both restorative and surgical criteria, within an objective and multidisciplinary framework.
Equally important are the patient’s expectations and the quality of doctor–patient communication, which play a central role in shaping the treatment plan. Medium- and long-term prognosis must be evaluated in both scenarios, taking into account biological, biomechanical, and esthetic factors, while certain clinical conditions may inevitably influence the direction of the decision.
This presentation will integrate personal clinical experience with current scientific evidence to provide participants with a comprehensive overview of the controversy and a practical decision tree to guide treatment choices in challenging cases.
